Letter of Appointment (LOA) - General Insurance
For MBAIS to act as your General Insurance Broker you need to formally appoint us by completing the letter of appointment below. Note;
- LOA's must be signed by a director, financial controller, or owner of the buinsess,
- The LOA must be printed on your business letterhead (if you do not have letterhead, please include your full name, business name and address at the top of the LOA),
- LOA's must be dated as they have a validity period.
